Introduction
Following the discovery of the charge-coupled device (CCD) in the late 1960s, CCD image sensors combined with image intensifiers (II) quickly became the defacto technology for dynamic digital medical imaging systems. CCD-II-based technology offered a superior option for dynamic imaging compared with the standard screen-film alternatives as the technology delivered significantly greater x-ray sensitivity at a much lower radiation dose. The synergy of high sensitivity and high conversion efficiency during fluoroscopy remains one of the biggest advantages of CCD-II technology and it is still widely used in medical c-arm systems today. It wasn’t until the arrival of flat panel detector (FPD) technology when CCD-II technology began being partially displaced. During the 2000s, innovative C-arm system manufacturers began integrating FPDs using amorphous silicon (a-Si) thin-film transistors (TFT) in their systems. FPD technology increased the utility of medical C-arm systems through improved image quality, better patient coverage from an increased field of view (FOV) as well as enhancing system usability and operator workflow. The lower electronic noise and reduced image distortion from a-Si TFT offers significantly improved image quality, especially when combined with powerful software algorithms for image reconstruction and optimization. In addition, the integration of an FPD over CCD-II requires smaller detector housing which reduces the size and profile of C-arm systems. This technology is now mature as a-Si TFT FPDs can be fabricated at low cost using the direct deposit of cesium iodide (CsI) which delivers better light collection efficiency and performance. Although a-Si TFT FPDs have many advantages the current generation of detectors continue to be limited by the performance of the TFT. The low electron mobility of a-Si requires large TFTs with high stray capacitance, which increases electronic noise and reduces the pixel fill factor. This negatively impacts the signal-to-noise ratio (SNR) and therefore inherently limits the image quality. In areas where a-Si TFT performance is constrained, complementary metal–oxide–semiconductor (CMOS) technology was introduced as it can offer material advantages in performance over a-Si TFT including improved resolution, electronic noise performance and speed. As a result, CMOS FPDs have also gained steady acceptance as system integrators and OEMs have re-evaluated which technology delivers an optimal balance between cost and performance for the clinical setting. Despite its performance advantages, the increased cost and complexity of manufacture has been a limiting factor for CMOS adoption. Since CMOS sensors are fabricated on wafers and therefore constrained to the wafer size, mechanical tiling of multiple CMOS sensors onto a fibre optic plate is required in order to achieve a larger field of view. Today, the majority of sales revenue generated from the sale of medical C-arm systems deploy an a-Si TFT or CMOS FPD as the market has gradually shifted from CCD-II technology. A sizeable market still exists for CCD-II based systems, primarily due to lower system price points, however, the ongoing market conversion to FPD technology has led several CCD manufacturers to discontinue production due to concerns around the future demand for CCD-II technology. This includes Sony during 2017 and most recently, ON Semiconductor (the former Kodak CCD business) during 2020. Further advancements in CMOS technology are anticipated to contribute toward further replacement of the CCD-II technology in the coming years. In addition, the recent commercialization of detector technology predicated on Indium gallium zinc oxide (IGZO), creates an environment where an accelerated replacement of CCD-II technology is plausible. IGZO technology has long been perceived as a high potential technology due to the potential to develop higher performance and lower cost TFT FPDs, relative to a-Si. IGZO TFTs have superior electron mobility over a-Si TFT’s which enables for a smaller size TFT while also reducing the pixel discharge time, resulting in improved electronic noise performance (SNR) and speed. A smaller TFT size also enables improved performance at low doses in dynamic imaging. Data from the first commercialized IGZO FPDs demonstrate that IGZO is an excellent technology for medical system integrators and OEMs to adopt across a spectrum of clinical settings and it is anticipated that the adoption of this technology will accelerate.
